Under indirect supervision, the Social Worker (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. The Social Worker (MSW) completes an Assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and apply discharge interventions to lower the patient risk of an undesirable event post discharge.
