Required– High School diploma or equivalent. Preferred – Phlebotomy certificate  CERTIFICATION Phlebotomy certification required within 18 months of hire. EXPERIENCE Preferred – Previous phlebotomy experience in hospital setting through previous employment or through phlebotomy externship with an approved program. SUMMARY Performs accurate identification of patient before collecting specimen or labeling specimen.  Performs venipuncture and dermal puncture, collecting quality specimen.  Collects quality specimens other than blood for testing (e.g. urine, DNA, fingerprints, swabs).  Accurately processes patient orders.  Accurately registers and enters courier work .  Performs additional tasks such as copying, fax, mail sorting, filing and data collection.

Graduation from an accredited nursing program; Bachelor's degree in nursing science (BSN), or current education plan to obtain BSN.   Preferred Bachelor's degree in nursing science from an accredited nursing program.   LICENSE/CERTIFICATION Registered Nurse Licensure, state of Indiana; CPR required; ACLS certification and PALS certification required within one year of hire.  CPAN or CAPA certification preferred.    EXPERIENCE Minimum Excellent customer service skills required. Demonstrated experience with requirements gathering, analysis and development of BI solutions. BI database application development and SQL experience required.  Must be proficient in ETL. Prioritize facility faxes and calls resulting in accurate, efficient responses to nursing homes, offices, pharmacies, etc. Accurate processing of physician and mileage payments.
