Under the direction of the Team Leader and within the Agency policies and procedures, the PTA follows the plan of care on individual patients within the guidelines of his/her licensure to provide for the safety and quality of care treatment or services.   Description:   1. Performs a comprehensive Physical Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history. Integrate pertinent data to develop a plan of care to promote function and safety. Participates with other staff in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care.   Description:   Maintain annual HHRG values at the National Standard as appropriate.
