Job Description

Responsibilities

The NP functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs). By using the SPs the NP is able to change treatment regimen after observing signs and symptoms of illness, reactions to treatment, general behavior and physical condition and determines if they are abnormal findings (Ca Nursing Practice Act, subsection(b) of Section 2725). The NP will provide and coordinate the care of GERIATRICS patients, provide patient/family education, promote safe continuity of care. The NP also participates in interdisciplinary and clinical rounds, provides education and support, as well as makes appropriate referrals and documents services rendered.

Qualifications

  • MSN/DNP in NP Specialty : Adult - Gerontology Primary Care, Adult-Gero Acute Care, or FNP -required
  • CA RN and CA NP license -required
  • BLS Certification from ARC or AHA -required
  • National Certification in NP Specialty- required
  • Ability to perform primary care responsibilities according to the Standardized Procedures for Nurse Practitioners as developed by the UCLA Division of Geriatrics and in accordance with policy- required
  • Working knowledge of Geriatrics and care of Geriatric patients - required
    Working knowledge of interdisciplinary and community-based resources needed to coordinate patient care and facilitate referral
  • Ability to take medical histories, perform physical examinations, assessments and implement appropriate diagnostic and therapeutic plans using medical practice in collaboration with supervising geriatrician culturally diverse environment
  • Knowledge of assessment and treatment of dementia, including psychological, behavioral, and medical aspects -required
  • Ability to monitor, evaluate. and improve dementia care - required 
  • Ability to provide education about dementia to patients and their families -required
  • Ability to assist in teaching programs for Physicians-in-Training and Nurse Practitioner -required
  • Knowledge of advance directives-required
  • Demonstrated ability to established and maintain professional cooperative and collaborative working relationships and project a professional demeanor while working with patients, peers, administrators, and physicians-required
  • Possess a high level of discretion in order to work with confidential information (i.e. patient’s medical history) -required
  • Demonstrated excellent writing skills to compose logical, detailed and grammatically correct correspondence procedures, clinical reports and recommendations- required
  • Demonstrated skill in establishing protocols- required
  • Ability to work independently and deal effectively with changing priorities-required
  • Ability to triage and manage problems as they arise on an as needed basis
  • Demonstrated ability to communicate effectively verbally and in writing to convey
    information accurately and precisely-required
  • Appropriate documentation that is in compliance with Medicare rule -required
  • Working knowledge of billing, particularly for incident to billing-required
  • Ability to administer clinical dementia assessments and dementia rating instruments -required
  • Ability to work collaboratively in a multidisciplinary team structure-required
  • Ability to develop collaborative relationships with primary care providers and community agency staff -required
  • Preferred;
    Working knowledge of UCLA policies and procedures.
    Bilingual Spanish speaking.
  • NP I: 6 months NP experience or less; NP2 : greater than 6 months experience
