Responds to the physical, mental, social and emotional needs of terminally ill patients and their families by providing skilled nursing care in their home under the supervision of a Registered Nurse or Physician. Reports to Hospice Administrator. Non-Exempt position.
Performs Licensed Vocational Nurse duties within the LVN scope of practice.
Provides palliative nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plan.
Functions as a team member demonstrating collaboration with and responsibility to patient, physician and interdisciplinary team.
Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses pain and notifies registered nurse when pain is not manageable on the current medications.
Performs patient care responsibilities considering needs to the standard of care for patient’s age.
Responds quickly and accurately to changes in condition or response to treatment.
Participates in Hospice’s other mandated activities as requested by the Director of Clinical Services including in-services and UR/PI committee.
Participates in on-call duties.
Communicates with RN care managers after patient visits.
Current state licensure as a LPN/LVN in the State of Texas required.
Current BCLS for healthcare professionals required.
One year LVN experience required.
One year prior hospice experience is preferred.
Valid state driver’s license, current automobile insurance and reliable automobile required.
Must possess superior ability to communicate effectively in English, both verbally and in writing. Additional languages preferred.
Excellent interpersonal skills and customer service.
Must possess high degree of organization and be able to multi-task efficiently and effectively.
Advanced computer knowledge required, including proficiency in Microsoft Office and Outlook.
Ability to use standard office equipment (copier, fax, etc.).
Functional Job Requirements:
Job requirements include, but are not limited to: sitting, standing, walking, kneeling, bending, squatting and reaching. Requires good finger dexterity, repetitive motions with hands and fingers. Requires ability to have good vision and hearing. Pushing and pulling equipment may be necessary.
Job requires heavy lifting on an occasional basis.
