SOCIAL WORKER – BEHAVIORAL HEALTH (2 SOUTH) Marietta Memorial Hospital, 401 Matthew Street, Marietta, Ohio, United States of America Req #2758 Thursday, June 23, 2022 Status: Full Time – 80 hours Hours: Monday-Friday - 8am-4:30pm   In an environment of continuous quality improvement, the Social Worker is responsible for performing social work and discharge planning services to patients and families to resolve social-emotional difficulties with illness and to ensure continuity of care for patients upon discharge. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.   Job Requirement: Bachelor’s Degree in Social Work required Master’s Degree in Social Work preferred Minimum of two years of appropriate experience preferred Licensed in the State of Ohio as a Social Worker Communication skills and ability to interact congenially and empathetically Job Functions: Provides emotional support, crisis intervention, and discharge planning services according to department procedures. Assesses and develops an appropriate plan for patients and/or families to assure continuity of care needs is met. Completes comprehensive assessments, interventions, and documentation is completed in the patient’s medical record according to written guidelines. Utilizes knowledge and skills necessary to assist with explanation and completion of Advance Directives. Assumes all other duties and responsibilities as necessary. Behavioral Health Additional: Manage all aspects of HBIPS (part of Quality Net) data auditing and collection including monthly audit of patient charts (EMR) and data entry into database. Report to director any potential shortcomings found. Manage patient information database and generates monthly reports to track and trend referral, admission and discharge data. Utilizes data to complete program’s key statistical reports and generates reports as needed. Initiate pre-certification calls for private insurance and managed Medicare and Medicaid plans. Also provide concurrent reviews throughout the patient stay. Track utilization review outcomes in binder. Manage insurance appeal process and work with hospital Denial Management Coordinator to make collection. Manages involuntary patients on a spreadsheet in regard to timing of filing court paperwork and hearings. Works closely with court personnel to maintain good relationship. Collects and manages all quality outcomes (like NCR Picker). Reports of program clinical successes submitted to Program Director. Prepares marketing collaterals and educational materials, including; mailers, fliers, brochures and presentations by formatting content and graphics. Coordinates with hospital marketing department to ensure required approvals are obtained prior to distribution. Write schedule for clinical staff.
