MaineGeneral Rehabilitation & Long Term Care at Gray Birch strives to provide the most comprehensive skilled nursing and rehabilitation services, long-term care and assisted living options to the southern Kennebec Valley community. The Ideal Candidate will have: Maine state licensure as Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) required. Certification as a Case Manager (CCM) by the Commission for Case Manager Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association (ACMA) preferred. Job Description: Determines, coordinates and supervises daily staffing assignments and levels. Provides direction, orientation, training, coaching, and mentoring to staff. Performs or assists with performance evaluations and disciplinary actions. Assesses quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies, procedures, and protocols. Performs staff responsibilities as needed to fulfill required service levels. Leads the handling and resolution of complex issues and complaints.
