  • Position Summary
    • The Population Health Care Coordinator is responsible for coordinating quality improvement efforts for physician services. The Coordinator works with the Operations Director, Physician Services Administration, and practice managers to spread best practices throughout physician services to ensure quality goals are met. The care coordinator is responsible for tasks assigned related to patient records in the Electronic Health Record (EHR) as well as managing patient panel reports. The care coordinator will serve as a resource to physicians and employees around process and clinical outcomes measures to maintain recognition through state and national quality improvement activities. The coordinator must be familiar with the processes underlying physician practice operations with a focus on demonstrating measurable outcomes, conducting all job responsibilities consistent with Southern Maine Health Care policies, procedures, and in compliance with all applicable laws or mandates relating to patient care and business activities.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: High School diploma preferred.
    • License/Certifications: Certificate from an accredited vocational institution or community college to indicate completion of a course in Medical Assisting is preferred.
    • Experience: Three years’ experience in systems and performance improvement and quality initiative in a physician practice setting preferred.
    • Knowledge of healthcare field and medical office tools protocols/procedures.
    • Extensive knowledge of physician practice operations required with the ability to maintain confidentiality.
    • Computer proficiency is a must with interest and experience in Microsoft Office, data collection, interpretation and web-based applications preferred.
    • Strong analytical and communication skills as well as strong interpersonal skills necessary for interacting with incoming callers and coworkers and the ability to work well with others.
    • Requires well organized and detail-oriented individual with the ability to organize and accomplish multiple activities in a fast-paced environment, have the flexibility to meet changing demands on a daily basis, and prioritize.
    • Must have the ability to maintain a professional manner even in stressful situations.
    • Must be creative and have ability to anticipate future challenges.
