Job Description

Summary

The Case Manager and Housing Support Specialist is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients to ensure residential stability and success for tenants in community-based Avesta housing.

The Case Manager will ensure that client service delivery practices are ethical, culturally-competent, and respectful of diversity and inclusion, welcoming and geared to customer service satisfaction, and will be compliant with regulatory, accreditation, contract and best practice standards.

Primary responsibilities of this role include:

  • Serve as the primary point of contact between Avesta and MBH services
  • Track and document data required for grants or by Avesta
  • Develop and implement outreach strategies in housing projects;
  • Support crisis intervention as needed to tenants at risk of homelessness
  • Make and document referrals to mental health, substance use treatment, child welfare, employment, education, and other services necessary to ensure residential stability;
  • Etc.

Behavioral Health Homes (BHH) are a MaineCare service designed to help you manage your mental and physical health needs. This service is available through community mental health providers such as Maine Behavioral Healthcare.

Avesta Housing is a nonprofit affordable housing provider with nearly 50 years of experience as a leader in affordable housing development and property management in southern Maine and New Hampshire.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: Preferred - Bachelor’s degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation or closely related field is required.
  • License/Certifications:  MHRT/C certification and CRMA may be required depending on scope of responsibilities; Eligibility for LSW licensure preferred;   Valid Driver’s license and appropriate level of automobile liability insurance
  • Experience:
  • 1+ year of Experience in casework and crisis intervention
  • Expertise in homelessness and housing issues
  • Experience in advocacy and working within community systems
  • Experience working with vulnerable populations
  • 2+ years of experience working with a broad spectrum of mentally ill clients is preferred
  • Prior experience with clinical methods and techniques used in supportive counseling as related to mental health and/or substance abuse issues is required
  • Excellent computer skills are required: Microsoft Word, Excel and use of electronic consumer record software
  • Knowledge and commitment to abide by the Code of Ethics of the National Association of Social Workers
  • Understanding of crisis intervention and support services
  • Ability to work both independently and collaboratively
  • Knowledge of community resources
  • Understanding of and positive regard for people living in poverty and the challenges they face
    • Ability to establish boundaries and set limits with a diverse population
    • Knowledge of casework theory and practice
    • Excellent communication skills, both oral and written
