Job Description

Summary

 

The MaineHealth Cancer Care Network is expanding patient support services.  Social work is central to providing psychosocial support and addressing barriers to care for patients newly diagnosed with cancer. Working collaboratively, the Social Worker will work closely with the health care systems within the MaineHealth network of hospitals and specifically with Stephens Memorial Hospital and Memorial Hospital. Social work will establish pathways for referrals and ensuring patients are cared for as close to home as possible. The Social Worker will actively identify and address barriers that might keep the patient from receiving timely, assure appropriate care for their cancer diagnosis and will connect patients with resources, health care and support services in their communities.  Oncology experience preferred, some travel is expected.

Position Summary

This is a position for the professional social worker that involves the evaluation of psychosocial needs of patients/clients and their families, and provision of diagnostic/preventive and treatment services including individual, family, and group therapy. Also provides complex discharge planning, coordinates aftercare services, and makes referrals for other community services based on the treatment plan. The employee works in collaboration with other health/mental health professionals and provides consultation to the treatment team regarding patient/family social and environmental needs. Work is performed under general supervision in accordance with institutional and social work/clinical counselor policies. May supervise other social workers, students, or other staff.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Two or more years work experience in a health or mental health setting preferred.

  • Knowledge of social work theories, techniques and therapies, as used in individual, family and group treatment, as well as principles of social welfare as acquired through completion of a Master's degree in social work from an institution accredited by the Council on Social Work Education.

  • Licensure by the Maine State Board of Social Work Licensure, LCSW.

  • Ability to communicate effectively and work collaborative with all members of the multidisciplinary healthcare team within Acute Psychiatry and Emergency Medicine
