Job Description

The Bed Coordinator will perform a multitude of duties to assist with the management of patient flow related to the patient’s bed assignment within the Tidelands Health system.  These duties will include being the central communication point and contact for bed management & placement for system.  The coordinator will effectively handle requests/calls by providing appropriate patient placement and correspondence back to the requesting department.  Additionally the coordinator will assist with monitoring and forecasting trends related to bed availability & needs for the utilization & planning of nursing/hospital resources. The coordinator will serve as an important role as a resource to assist with the continuous improvement & effectiveness of bed management function and related metrics for the Tidelands Health System. The Bed Coordinator must be able to effectively communicate and collaborate with a variety of clinical referral resources/departments including physician groups, senior leadership and consulting facilities daily.  As part of the bed management process the coordinator must possess excellent decision making skills with the ability provide resolution according to the Tidelands Heath policies & procedures. Detailed Responsibilities include: Collaborates with hospital staff, case management, physicians and their staff to define, reduce and eliminate barriers within the system which may impede patient flow. Provide effective communication with team members and other departments to ensure seamless process. Coordinates daily bed/staffing huddles to assess discharges and admissions for the facility. Responsible for timely bed assignments and prioritizes admissions from multiple areas ensuring patients are placed quickly and accurately. Assigns patients to appropriate bed utilizing designated patient placement guidelines. Applies knowledge of hospital and unit admission criteria and type of care provided in each department to appropriately place patients. Demonstrates competence in current knowledge of specific unit placement criteria. Reviews pertinent clinical information for appropriate placement. Coordinates interdepartmental transfers and discharges. Evaluates appropriate patient placement, prioritizing according to current census, staffing situations and knowledge of patient acuity. Receives and obtains accurate and complete admission information from physicians, physician office staff, and outside facilities for patient admission and placement. Acts as a liaison to outside physician offices. Will keep in constant communication with outside referral sources to facilitate the direct admission process. Bed Tracker, admissions logs, and Red Flow data collection sheets are kept accurately, are update and are completed efficiently and timely through prioritization. Works closely with unit managers to problem solve issues related to admissions, transfers and discharges. Inputs information with 99% accuracy Obtains accurate and records complete information regarding a patient’s death Acts as a resource for corners needing information about patients Acts as a liaison between the hospitals and funeral home and notifies when body is ready for pickup. Acts as a resource for co-workers as well as other hospital employees Works on departmental performance improvement projects as assigned. Supports facility performance improvement activities as assigned i.e. completing audits, data collection, etc... Attends monthly department staff meetings to give input and stay up to date QUALIFICATIONS   Experience:  Previous health care and customer service experience preferred.   Education: High School diploma or equivalent, required.   Special Skills:  Must be well organized and detail oriented. Working knowledge of word, and excel. Experience with chart and data extraction. Knowledge of hospital admission criteria. Excellent interpersonal and communication skills. Critical thinking and problem solving skills in a high stress and fast paced environment. Knowledge and experience in human relations and conflict resolution. Incorporates knowledge of unit specialties, limitations, and patient needs when determining patient placement Provides feedback to hospital management regarding staff actions/behaviors effecting patient throughout.   Physical Requirements: Light-Medium While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear. The employee must exert 15 to 20 pounds of force occasionally (activity or condition exists up to 1/3 of the time), and/or 5 to 10 pounds of force frequently, and/or greater than negligible up to 5 pounds of force constantly to move objects.   The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
