The Wound RN Case Manager works with the physicians to coordinate the continuation of patient care and provide patient care with wound healing. Responsible for services such as setting up home health, ordering DME, reports to referring physicians, and data base entry. This position requires the case management of up to 40 patients. Duties include patient chart reviews and other compliance review activities to assist the department. Will also serve as the safety officer of the clinic and in general working with the clinic as a team member to provide optimum patient care services and exemplarily overall customer service as well as complete other duties as assigned. If HBO certification is held, is required to manage the hyperbaric oxygen therapy treatment care of all HBO patients as well as responsibility for all HBO competencies and education of peer RN Case Manager staff.
Associates Degree in Nursing (ADN), required.
Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN), preferred.
Degree received from an education institution accredited by National League of Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE), preferred.
RN with case management and wound care experience, preferred.
Previous experience and/or hyperbaric oxygen therapy (HBO) competencies, preferred.
Current and in good standing RN Licensure with South Carolina Board of Nursing, required.
American Heart Association Healthcare provider course with Basic Life Support (BLS) certification required to be initially completed with the TH Organizational Development Office within 30 calendar days of hire date (exceptions to obtain certification from a provider other than the TH Organizational Development Office are approved by TH Organizational Development on a case by case basis). BLS Recertification is required every two (2) years.
Works with patients, families, physicians, and other health care providers.
Microsoft Office Suite, data base entry, and other computer skills, required.
