CARE MANAGEMENT COORDINATOR: The Care Management Coordinator is responsible for overseeing the processes and documentation that occur as evidence based practice is applied at the bedside. Directs and or completes all functions of the government and commercial utilization management process. Assesses and coordinates implementation of the discharge plan. Collaborates with the multidisciplinary team to assure effective, efficient, appropriate care in order to optimize the health status and assure a smooth transition between levels of care for patients served. Requires weekend, holiday, and off site commitment.
JOB DUTIES: 7A- 7:30PM, rotates every 3rd weekend, 1 summer and one winter holiday.Average about 20 patients each
• Initial Psychosocial Assessment
• Set up home care
• Set up DME (Durable Medical Equipment) for home
• Set up Tube Feeding/ TPN/ IV's
• Set home oxygen
• Set up home Wound Vac's
• Set up Outpatient Infusion for Antibiotics
• Set up Transportation for Home Care patients
• Assist with set up of Assistant Living (not too often though)
• Get patient signature for required Medicare Rights and Observation Status notification
EDUCATION: Associate's Degree or Diploma Nursing Hires into position (internal and external) required to obtain BSN within 5 years of hire. Registered Nurse EXPERIENCE: 3-4 years Clinical in Nursing
SPECIALIZED KNOWLEDGE: Excellent communication skills with the ability to negotiate, problem solve, & establish effective working relationship with members of the healthcare team and external customers. Knowledge & experience in computer key boarding.
