RN Care Manager (registered Nurse)- Elmwood Primary Care

The RN Care Manager provides care management services to promote continuity of care and optimal patient outcomes to patients who are at most risk for health deterioration, complex chronic disease and/or poor outcomes.
The Ideal Candidate Will Have
Bachelor's degree, BSN preferred.
Associate's degree, Nursing required.
Must have either a current State of Maine Registered Nurse (RN) license or an RN license recognized by the Nurse Licensure Compact (https://www.ncsbn.org/nlc.htm). If RN licensure is from a compact state, must obtain a state of Maine RN license within 90 days of changing primary residency to Maine required.
3 years or more preferred.
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines,prioritizes, provides and/or arranges for needed internal and external services/interventions.Consults with multidisciplinary teams regularly to evaluate patient's status and appropriateness of medicalcare, including admission, length of stay, transfer and discharge.Participates in discharge planning including coordinating patient transfers to other facilities and coordinatingcommunity resources. Provides discharge education and resource referrals to patients.Performs chart review to identify actual or potential issues with service delivery, patient outcomes andsatisfaction, compliance, cost, and reimbursement.
