To provide appropriate and timely coordination of and arrangements for follow-up health care, assessment for financial resources available for patients, and referrals to appropriate community resources as needed.
Assisting with behavioral assessments & discharge planning for the behavioral health patients of LCMH's Behavioral Health Department. The Case Manager coordinates with the Treatment Team and when necessary completes assessments to ensure patient is receiving quality treatment planning, discharge planning or aftercare referrals. Specifically, the Case Manager collects history and summarizing, provides patient and family education, and implementation and discharge planning of patient’s aftercare or referral resources. The Case Manager serves as a patient advocate and participates in the multi-disciplinary approach to meeting the needs of patients, their family &/ or their natural support system. Assists in the identification of patient needs and education on how to effectively socialize, communicate, relax and manage their stress by means of utilizing healthy outlets and coping skills; Demonstrates a caring attitude by practicing guest relations, promoting a positive, professional and cooperative work environment. Is a member of the Behavioral Health Departments Service Excellence Committee.
BS or BA in Social Work, Sociology, Psychology, or Criminal Justice preferred.
One year experience in social service field.
Ability to communicate with patient, family, peers, and medical personnel and community referral agencies.
Articulate understanding of and willingness to support the philosophy, the objectives and the policies promoted within the hospital.
