Summary

The Assertive Community Treatment (ACT) programs of Maine Behavioral Healthcare are looking for a Case Manager to assist in the provision of case management services and achievement of positive intervention outcomes for assigned clients. He/She will ensure that client service delivery practices are ethical, culturally-competent, respectful of diversity and inclusion, welcoming and geared to customer service satisfaction. The Case Manager will be compliant with regulatory, accreditation, contract and best practice standards.   Assertive Community Treatment (ACT) is an intensive community based (outpatient) treatment program with a multi-disciplinary team of providers that supports eligible adults and children who may have a major mental illness diagnosis and often co-occurring disorders. The goal of the ACT Program is to improve the client’s functioning in the least restrictive setting, while strengthening family, work, school and community ties.   As an ACT Case Manager, your responsibilities will include:
  • Effectively coordinate service requirements and referral of clients to appropriate community resources

  • Ensure effective communication in a secondary language when required to ensure the comparable availability of agency services to special client populations

  • Ensure the timely and effective crisis and safety planning for each assigned client

  • Participate in discharge planning and case closing consistent with agency standards and deadlines

  • Provide clients/family education: clients rights and responsibilities, help families cope and build strong resources and support for family members

  • Provide transportation and accompany clients to appointments as deemed necessary 

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: Bachelor’s degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or closely related field is required. 1+ year of experience in the same areas of specialization
  • License/Certifications: Depending on scope of position, MHRT-C certification may be required. Eligibility for LSW licensure preferred Valid Driver’s license and appropriate level of automobile liability insurance.
  • Experience: 2+ years of experience working with a broad spectrum of mentally ill clients is preferred. See also "Education"
  • Excellent computer skills are required: Microsoft Word, Excel and use of electronic consumer record software
