PHARMACIST-LGMC PHARMACY-PRN (JOB ID: 5641)

The Pharmacist assumes the responsibility for providing services to in-patients, out-patients, and employees.Staff Pharmacist is responsible for compounding and dispensing all drugs, including I.V. mixtures and hyper alimentation solutions, monitor patient drug therapies, and provide drug information to doctors, nurses, and patients as requested.The Pharmacist shall report directly to the Director of Pharmacy and will supervise all pharmacy support personnel in the absence of the Director.
A.The Pharmacist provides functional supervision of the Pharmacy services by:
a.Allocating and coordinating the work flow ensuring pharmacy services are provided in a timely manner.
b.Supervises and directs pharmacy support personnel, advising support personnel in the more complex phases of their work.
c.Answers the telephone, identifying self and department, directing calls to appropriate personnel.
d.Keeps pharmacy areas and equipment clean, neat, and well-organized.
e.Training of new employees.
B.The Pharmacist prepares and dispenses medication orders per physician requests according to established policies, procedures and protocols by:
a.Interpreting medication orders (written and verbal) and transcribes to computerized patient medication profiles accurately. Maintains accurate and complete medication profiles.
b.Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately
c.Issues controlled substances to patient care areas and maintains records as required by law.
C.The Pharmacist ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures and protocols by:
a.Monitoring drug therapy regimens for contraindications, drug-drug interactions, drug-laboratory test interactions, drug-food interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
b.Assist with pharmacokinetics consult service and drug dosing per organizational protocol.
c.Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.
d.Reads, extracts, and interprets information in patient charts accurately.
e.Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.
f.Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
g.Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
h.Provides accurate, adequate, and timely drug information to the professional staff.
i.Provides drug information to patients and their families as needed.
j.Documents all clinical activities and interventions accurately and completely.
k.Participates in the quality improvement and medication use review activities of the department. Collects data, conducts quality monitors and inspections and maintains log, records, and other documentation as assigned.
D.The Pharmacist maintains competence required for current job title/description by:
a.Maintain current pharmacy licensure
b.Attend department staff meetings
c.Attends orientation, education, and training programs.
d.Reviews literature and other materials pertinent to the practice of pharmacy including but not limited to new drugs, improved technology, and changes in laws.
E.The Pharmacist assumes additional responsibilities as requested.
1.Pharmacist licensed as set forth by the State of Louisiana required
2.Health system experience preferred, but not required.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
Medium Work as defined by the U.S. Department of Labor constitutes a maximum lift of 21-50 pounds on occasion and/or a maximum lift of 11-25 pounds on a frequent basis
