We treat each employee, patient and resident like we would a member of our own family, and measure our success by the direct impact we have on improving the quality of life for our employees, patients and residents.   Responsibilities: Under the direct supervision of the Wound Care Registered Nurse or the Chief Nursing Officer designee, provides patient with care that is continuously evaluated, planned for, and revised as needed.  The LVN/LPN, RN, or Physical Therapist Wound Care Nurse/Therapist conducts all required wound care activities according to hospital policy and national standards.  Collects the data from a comprehensive wound assessments and carries out the plan of care including goals, implementation, evaluation of outcomes for patients ranging in age from 18 years to geriatric.  Provides patient, family and staff education regarding prevention of pressure ulcers and therapeutic support services, dressing selection and adjunctive therapy.  Collaborates with clinical staff, physicians and other members of the treatment team to provide the patient with the highest quality and most appropriate wound care.  Acts as a resource for the treatment team and clinical staff and assists with staffing as assigned.  Trains and manages the Wound Care Team Members to include orientation, and in-service training.  Coordinates with the  interdisciplinary Wound Care Committee as applicable. Qualifications Minimum Qualifications: Projects a professional image. Knowledge of regulatory standards and compliance requirements. Working knowledge and ability to apply professional standards of practice in job situations. Strong organizational and analytical skills. Knowledge of comprehensive wound management including data collection, recommendation and application of appropriate wound dressing, documentation, education, and surveillance and outcome data. Strong organizational skills. Working knowledge of personal computer and software applications used in job functions. Education and/or Experience:  Current state license as RN, LPN or Physical Therapist in good standing. Two or more years of acute care or rehabilitation. Two or more years of wound care experience is preferred, including collection of wound data, recommendation of treatment plan, recommendation of appropriate wound dressing, documentation, , education, data collection, surveillance and outcome data. Knowledge of principals of infection control.    Certificates, Licenses, Registrations:  Current state license as RN, LPN or Physical Therapist in good standing.
