This position will support our outpatient practices at Southern Maine Health Care in Biddeford, Maine.
The Population Health Manager is responsible for the operational oversight of the Population Health program at Maine Medical Partners.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Minimum of a Bachelor's degree in Nursing or Social Work from an accredited institution. Master’s Degree in health related field preferred. 1.
License/Certifications: Current, unencumbered Maine RN license OR Licensed by the Maine State Board of Social Work required
Experience: Prior population health experience preferred. Minimum of 3 years' ambulatory care experience required.
Mastery of theories, principles and methodologies underlying population health and the patient centered medical home preferred.
Strong background in health promotion, chronic condition management, care coordination, motivational interviewing, care/case management and care coordination preferred.
