Job Description

This position is responsible for providing nursing care in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Le Sueur Medical Center and the Minnesota Nurse Practice Act. Job Functions  Collects and monitors patient health data and provides patient care. Advises and educates patients, family members, and significant others on health maintenance and disease prevention for the patient. Documents and maintains accurate medical records and reports. Serves as a patient advocate in seeking appropriate community resources to meet patient needs. Demonstrates competencies in the specialty area of Long Term Care. Performs other duties as assigned. When Functioning as a Charge Nurse: Assigns resources to care for patients and arranges staffing to ensure safe patient care. Provides work direction and addresses issues during the shift. Acts as a liaison between staff, patients, and physicians. Demonstrates unit specific Charge Nurse competnencies. Reviews physician oriders. Minimum Education/Work Experience  Graduate of an accredited school of practical nursing Knowledge/Skills/Abilities Demonstrate critical thinking and decision making Ability to meet and maintain the necessary background checks as aligned with position functions Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above Ability to work autonomously and as a team Capable of making independent decisions with the use of good critical thinking and problem solving skills within the scope of practice Ability to work and multi-task in a high stress and fast paced environment Ability to function both independently and as part of a team Ability to respond appropriately to unpredictable situations and diverse patient populations Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above License/Certifications  Current Practical Nurse licensure in Minnesota or if licensed in another state, a permit from the Minnesota Board of Nursing while the license application is in process Healthcare CPR Certification Preferred Qualifications  Previous nursing experience in a geriatric or nursing home setting Cognitive and Mental Demands  Time Awareness - Continuous Writing - Continuous Initiation of Work - Continuous Memory - Continuous Receptiveness to Change - Frequent Reasoning - Continuous Required Sequences in Work - Occasional Problem Solving - Continuous Attentiveness Duration - Continuous Social Interactions - Continuous Mathematical Aptitude - Frequent Conflict Management - Continuous Reading - Continuous Detail Oriented - Continuous Link to Employee Health Physical Demands   Additional Information FTE: .8 Weekend Rotation: Every Third Weekend Holiday Rotation: Every Other Holiday Primary Shift: Evening Department: Nursing Home and Rehab Center
