The medical Social Worker assists the physician and other team members in understanding the significant social and emotional factors related to the health problems.
Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to case manger and clinical manager.
Carries out social evaluations and plans intervention based on evaluation findings.
Provides information and referral services for patients and families/caregivers and coordinates community resources regarding practical and environmental needs.
Participates in the development of the plan of care and case conferences.
Assists physicians and other team members in understanding patients significant social and emotional factors related to health problems.
Maintains clinical records on all patients which include initial evaluations and treatment plans; daily progress notes which reflect patient status and progress toward long and short term goals; interim re-evaluations showing treatment plan adjustments; case conference reports; and discharge summaries.
Attends educational programs and professional meetings as they relate to the provision of social work services.
