The Medical Weight Loss Coordinator is a Nurse Practitioner or Physician Assistant who delivers and coordinates care for patients in the Metabolic and Medical Weight Management Program. They report directly to the Medical Weight Loss Medical Director. Under their general supervision, this position renders diagnostic and therapeutic care to patients to include clinical practice, education, consultation, research, and program coordination. This role is a member of a collaborative team of physicians, providers and clinicians working to provide compassionate care to include evaluations and assessments, diagnosing, prescribing, treating and educating patients.
The Medical Weight Loss Coordinator will develop, promote and advance the non-surgical aspects of the Weight Loss Program across a multi-facility integrated healthcare delivery network. A positive, compassionate and outgoing communication style is a key success factor. Strong interpersonal skills and comfort in speaking on subject matter with patients, physicians and colleagues is essential.
In addition to program coordination, the Medical Weight Loss Coordinator shall collaboratively work to ensure regulatory and accreditation compliance and smooth transitions across service lines. They work closely with Medical Directors, Surgeons, Administrator and Marketing. This position will require a high degree of self-initiative, collaboration and teamwork.
Education and Experience:
If Nurse Practitioner, must be a graduate of an accredited school of nursing with successful completion of an approved Nurse Practitioner program. If Physician Assistant, bachelor’s degree and successful completion of an accredited physician assistant program required. Minimum of 2 years of experience working independently with patients in family practice, weight management, functional medicine or similar specialty.
National certification from the National Commission of Certification of Physician Assistants. State PA license required. Current DEA certificate. -OR- Minnesota Nurse Practitioner license. Current American Heart Association approved BLS certification. Certification in Diabetes Management and/or Obesity Management.

Obtain patient histories and perform physical examinations on patients.
Order appropriate diagnostic tests and labs.
Diagnose and treat health conditions related to Medical Weight Loss.
Prescribe pharmacological agents and non-pharmacological therapies.
Plan a course of treatment with patient.
Consult with physicians as needed and initiate referrals as appropriate.
Complete all documentation timely, efficiently and in its entirety.
Partner with PCP, referring physician or surgical specialist to assure best patient experience and outcome.
Manage the health care of those patients for which they have been education to provide health teaching and counseling.
Maintain health record and document all items required for Meaningful Use.
Accurately complete all patient notes within 48 hours and forward completed documentation to supervising provider for signature.
Address and complete all patient tasks daily.
Assist supervising provider with completion of their tasks and assist with paperwork and forms as directed by the physician.
Return patient calls as needed.
Provide comprehensive and continuing care to patients requiring treatment through the diagnosis of disease processes and through the provision of medical treatment of patients.
Serve as the Provider for CRMC’s Medical Weight Loss program.
Provide services in support of health promotion and preventative medicine in order to maintain a holistic care model for all patients.
Provide support and guidance to other health care professionals such as staff nurses and health care staff to ensure there is proper delivery of medical care to patients.
Ensure patient confidentiality is maintained at all times.
Participate in patient care audits for quality assurance and utilization review to maintain and improve patient care standard of excellence.
Track patient progress and assist patient in meeting personal goals to support patient and meet organizational quality metric goals.
Maintain medical professional standards and knowledge by regularly attending medical conferences, seminars, continuing education programs, and teaching medical residents, medical students, and/or practitioners or students in other healthcare professions.
Provide support to the team by maintaining open channels of communication to create and maintain a culture of transparency and accountability.
Perform other duties as assigned.
