The Registered Nurse (RN) is responsible for providing comprehensive nursing care for patients and families through effective use of the nursing process and by following established policies and protocols. This position exercises independent judgement to assess patient needs and works collaboratively with the Medical Staff, Providers, Nursing Staff, Administration, and Support Services to provide safe, quality care to patients. This position will be able to triage patient calls, conduct clinical calls to patients (post ED/IP discharge and return calls) under the advisement of Providers and assist Providers in patient care when needed, provide education to patients, coordinate care among an integrated care team, specialists and community resources as well as identify high and rising risk patients and alert the care team. Responsible for supporting primary care objectives including outreach to patients, surveillance of health maintenance areas, coordination of care, triaging and proactively creating relationships with Providers and patients to ensure optimum health and wellness. This role is primarily administrative with limited patient contact.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Current licensure as a Registered Nurse in the State of Maine.
CPR certification required.
Must be self-directed, highly motivated and highly productive.
Excellent computer skills, strong telephone skills and public communication skills.
Experience in a primary care setting preferred.
Strong organizational skills and ability to prioritize workload and maintain a flexible problem solving approach.
