The Case Manager promotes comprehensive and continuing patient care by providing appropriate discharge planning to patients and families.
Completes patient assessments and referrals in a timely and expeditious manner. Makes appropriate assessment of patient needs, monitors for changes, and changes plan as needed to ensure the appropriate continuum of care, in such a manner to
expedite a timely, effective discharge plan for the patient.
Completes accurate insurance reivews. Performs commercial insurance reviews. Secures information from commercial insurance providers as to the preferred providers of post hospital services, and review with patients, in situations where post hospital services are needed.
Facilitates appropriate discharge arrangements as deemed appropriate with physician approval. Serves as a member of the interdisciplinary discharge team.
Assists the patient and family with financial difficulties in location of possible resources. Is knowledgeable as to community resources, attempts to remain updated, and attempts to locate new resource information. Provides supportive resource information to help patients and families cope with problems.
Acts as an advocate for patients for all their human rights and makes referrals for more specific advocacy. Acts as a liaison to community agencies in cases of suspected abuse and neglect, recognizing signs and symptoms of abuse and neglect, and is knowledgeable of the applicable policies and procedures.
Completes all departmental activities, including hospital meetings, handling correspondence, maintenance of statistical data, participating in educational programs, safety programs, and other duties as assigned by the Director of Case Management.
Provides for the coordination and provision of needed support services, which may be necessary to facilitate appropriate discharge plans.
Establishes and maintains good working relationships with other staff, agencies and organizations.
