Job Description

Position Summary:    The Physical Therapist reports to the Manager of Rehabilitation Services and is responsible to provide quality patient care per the plan of care upon the referral from an appropriate licensed practitioner or via direct access per state laws.  The Physical Therapist is responsible for developing a plan of care specific for each patient and for implementation or modification of the plan, as necessary.  Documentation is completed as per department policy.  Documentation supports billing; billing is done accurately. The Physical Therapist may delegate and/or supervise appropriate aspects of care to the Physical Therapist Assistant or Rehabilitation Aide.  The Physical Therapist will perform job duties is an efficient manner meeting productivity targets and adhering to very good patient satisfaction principles.   Essential Job functions and applicable professional competencies:   The Physical Therapist will perform high quality evaluations which include interpretation of physician referral, accurate history, collection of objective data and proper documentation and record-keeping to support billing.  Documentation completed in timely manner, functional and objective goals, utilization of objective tools for testing  billing accurate per chart audit, meets guidelines for required supervision and Occupational Therapist intervention for progress updates during POC, efficient use of SOARIAN.    The Physical Therapist will perform high quality treatments which include proper documentation, record-keeping and billing.  Documentation completed in timely manner, functional and objective goals, utilization of objective tools for testing, billing accurate per chart audit, meets guidelines for required supervision and Physical Therapist intervention for progress updates during POC, efficient use of SOARIAN.  The Physical Therapist may delegate and/or supervise appropriate aspects of care to the Physical Therapist Assistant or Rehabilitation Aide.        Staff member performs 4 peer reviews per month. Provides appropriate delegation to and supervision of Physical Therapist Assistant or Rehabilitation Aide.  Provides opportunity for student learning and completes paperwork for clinical affiliations. Provides opportunity for student learning opportunities and completes paperwork for clinical affiliations as required by academic institution.     Effectively utilize the time, talents and other departmental resources to meet daily minimum productivity target (8 hour prorated) of 24 UOS in acute care* and 26 UOS in CCR and OP.  Productivity targets achieved. Promotes environment of very good patient care by welcoming patient and adhering to AIDET.  Demos that patient satisfaction is a priority by supporting achievement of Press Ganey target and/or HCAPS/VBP targets. Participates and initiates activities or programs that promote education and development of self, peers and community.  Actively participates in staff meetings, attends annual CEU offering at facility, provides in-service for any CEU event attended in community, participates or presents educational information in community events. Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns.  Complete patient or employee safety reports as appropriate, reports equipment that needs service, suggest safer techniques for patient care.        Other duties as assigned   Education and Experience:    Bachelor Degree required. Degree in Physical Therapy, current requirement is Doctorate of PT for new graduates but pending graduation Bachelors or Master Degree is acceptable. State License in Physical Therapy.  BLS certification required. Ability to communicate with diverse patient population to allow insightful gathering  of information, testing and education.   Written and verbal skills to document patient progress and/or speak with referral sources or third party payers.  Time management skills.  Computer skills to facilitate training on required software for electronic documentation, billing, etc.  Prioritization and delegation skills.
