Bluegrass Care Navigators connects patients with caregivers who have exactly the right expertise to meet their health care needs and help handle the activities of everyday living. Our team provides unsurpassed private duty nursing, palliative care, hospice care and more. Position Qualifications: Is a graduate of an accredited school of professional (registered) nursing. Active KY state RN license At least two year’s general nursing experience is required with current hospice, home health, and medical, surgical or critical care experience. Has experience with providing good pain management. Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Complies with accepted professional standards and principles. Understands Hospice philosophy, principles of death/dying. Understands Medicare Conditions of Participation of a hospice program and all applicable rules and regulations. CPR certification, negative TB skin test, pre-employment physician’s health clearance Demonstrated skill in administrative performance that includes expertise with communication, both written and oral. Public speaking skills. Must have the ability to work independent of direct on-site supervision. Demonstrated ability to work well with health care professionals and an interdisciplinary team. Must arrange travel and be able to access hospitals, nursing homes, and other places of business. Current driver's license and auto liability coverage Summary of Job Responsibilities: Educate hospital professional and non-professional staff regarding Hospice of the Bluegrass services and programs. Facilitates communication between Hospice of the Bluegrass and physicians in an inpatient setting. Manages the development, implementation, monitoring, and planning and supervision of strategies to increase referrals. Assists with referrals to Bluegrass Hospice Care. Provides oversight/case management of hospitalized hospice patients. Why Bluegrass Care Navigators?
