JOB SUMMARY OF RESPONSIBILITIES
The Occupational Therapist Case Manager is responsible for case management and primary patient care for patients in an assigned geographic area.
Provides leadership for the Triad Care Team with direction to OT staff therapists, COTA’s and participates in interdisciplinary coordination of patient care. Provides field supervision of CNA staff when appropriate. Works independently with referral-intake-scheduling-authorization (RISA), staff therapy management and practice support services.
MINIMAL JOB QUALIFICATIONS
Prefer at least three years’ experience as a Registered/Licensed Occupational Therapist in a hospital, outpatient, or health care setting. Must have at least 1- year experience in hospital or outpatient setting and 1-year experience in Home Health.
Graduate of an accredited program in Occupational Therapy.
