It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Individuals enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home, often times without ever having to go into a nursing home.   The focus of every PACE organization is to help the elderly live in the community for as long as possible. To meet this goal, PACE organizations focus on preventive care. Bathing Assistance Valir PACE understands that older adults often struggle with bathing tasks, which can make maintaining hygiene difficult, if not impossible.   If this situation sounds familiar, let us help with things like getting in and out of the shower, washing hair, and any other bathing assistance as needed. With our help, your loved one can maintain proper hygiene without losing independence.   Light Meal Preparation Meal preparation is another household task older adults often have difficulty with, and one we’re ready and willing to make easier.   Our Home Care team is experienced at putting together simple, nutritious meals and snacks to ensure our program participants receive their caloric intake and nutritional needs at home. Qualifications Responsible for the provision of activities of daily living as identified by the Home Care Coordinator on the plan of care.   POSITION REQUIREMENTS: Licenses or Certifications: Home Health Aide certification Total Education, Vocational Training, and Experience: HS Diploma or equivalent with certification as a Nursing Assistant. Certified to pass Home Health Aide Competency Test and Skilled Competency Checklist. One year of Home Health Aide or LTCF experience. Minimum one year experience working with a frail, elderly population. SKILLS AND ABILITIES: Exhibits caring attitude toward patients and families. Must be mature and able to cope with the demands of the job. Bilingual in Spanish or Vietnamese is beneficial. PROTECTED HEALTH INFORMATION: Position has Full Access to Protected Health Information of patients on caseload.
