TITLE: Transitions Care Coach
Kentucky licensed RN or LPN pr graduate of Master's or Bachelor's program in Social Work.
Minimum 1 - 2 years experience in home health, health care or community based organization.
Strong communication and organizational skills.
Knowledge of medical terminology.
Strong education/teaching background.
Ability to be flexible and adjust practice based on client population being served.
Experience in problem solving, managing complex problems and developing solutions.
Strong customer service skills and the ability to work independently.
Ability to exercise judgement and discretion in confidential matters.
Must be able to make home visits to patients in a variety of settings: mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.
Must be able to drive for extended periods of time with no ill effects.
Must have flexible schedule.
Good written and oral communication skills
Must have computer and email access and excellent computer skills.
Must have the ability to give and receive constructive criticism.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Possesses and maintains current CPR certification (online CPR certification is not acceptable).
SUMMARY OF JOB RESPONSIBILITIES
Transitions Care works in partnership with physician groups and hospitals to reduce hospitalizations, hospital re-admissions and emergency room visits among high-risk patients with chronic conditions through in-home care management provided by a specially trained team of licensed nurses.
The Transition Care Coach will be responsible for providing coaching services to eligible patients in various environments and locations. The staff will develop a wellness coaching relationship with clients and assist the client towards better health, by providing support, encouragement, education and resources when needed.
