:: The Diabetes & Nutrition Teaching Center of Aiken Regional Medical Centers is seeking an RN-CDE (Certified Diabetes educator) to join our American Diabetes Association accredited team for a fulltime M-F position. � The basic role will require the CDE to assess all educational needs and provide diabetes self-management training to outpatients and their families/caregivers. Education is provided to those with diabetes of all types and all ages both in individual and group settings. The CDE must possess knowledge of patient needs across the life span and use critical thinking skills to identify the patient's status and educational needs using clinical data and patient assessment. The CDE participates in the development of education records and teaching materials and works in collaboration with referring physicians and disciplines as required to promote and provide quality care to persons with diabetes.
:: Essential Requirements: � Performs initial and subsequent patient contacts Patient scheduling and direct intake Coordination of patient registration Conducts a thorough, individualized self-management assessment of the patient. Performs assessments of physical health, medications, and psychosocial issues and identifies areas for education and clinical interventions. Guides the patient with diabetes in setting individualized behavioral and clinical goals. Develop success metrics and use behavior change methodology to facilitate patient participation in the education process. Assist develop and update education plans to address behavioral goals established during the goal setting process, develops a learning plan to address gaps in knowledge, collaborates with patients to plan strategies for addressing barriers identified, and refers to prescriber as needed. Continually assess the progress of the plan and the patient?s understanding of goals and plan, knowledge, and clinical and behavioral goal achievement. Must be an effective communicator. Relays concepts and instructions to patient in a clear and concise manner with information/materials based on the patients specific needs. Contacts patient family members and includes them in the learning process as indicated and documented in the patient record. Maintains patient records of all materials used and taught including physician order, time spent, teaching plan, plan revisions and follow-up. Acts as a liaison with patients, physicians, departments, and other healthcare professionals as required. Participates in departmental activities related to performance improvement and quality control. Assist with public events and perform public Diabetes presentations. Responds appropriately in an emergency situation and assists promptly and professional when necessary. Annually, obtains the required 15 hours of continuing education related to diabetes management-Required All other duties as assigned. Experience:- 3 ? 5 years Required: Education ?
