Regions Hospital is seeking an ED Crisis Care Coordinator. The case manager works as part of the health care team to identify and collaborate with the assigned hospital social worker in providing social services to patients and families at Regions Hospital. This position utilizes a full range of social work treatment modalities based on social work assessment/ and interventions. Facilitates communication between staff, patients, families, care providers, and between the inpatient unit and other hospital departments. Directs patient flow from ED to inpatient units. Serves as a resource person to ED staff regarding hospital protocols and mental health care. Simultaneously manages many activities impacting direct patient care. Assists social worker, in providing education and leadership to other health care professionals in the psychosocial aspects of patient care. Provides exceptional customer service to patients and visitors.
Holds a bachelor’s or higher degree in one of the behavioral sciences or related fields from an accredited college or university or holds a bachelor's degree in other field and has at least 2,080 hours of supervised experience in the delivery of services to persons with mental illness or related/applicable experience. At least two years of experience in mental health care and/or acute health care or related social service setting. Must hold a Human Services degree, or be a MN Board of Social Work, licensed LSW or LGSW. (LGSW must be under direct supervision of an LICSW).
