Functioning within the Hospital’s mission, values, objectives, policies and procedures, the Medication History Technician works independently with other health care providers to gather information and compile accurate home medication lists for patients being admitted to the hospital. This information is collected from sources which include, but are not limited to, retail pharmacies, physician offices, mail order pharmacies, electronic resources, and patients/families. The Medication Reconciliation Technician will use all available information sources to obtain a medication list which is as accurate as possible. Once compiled, the medication list will be entered into the Hospital Information System (HIS) for use by providers to reconcile the medications and perform Computerized Provider Order Entry (CPOE).
Education: High school diploma or equivalent is required with educational background that includes some clerical, science (chemistry preferred), and mathematics courses.
Licensure/Certification: Certification by the Pharmacy Technician Certification Board (PTCB) is required within one year of date of hire.
Experience: One year of previous experience in retail pharmacy setting is preferred.
Skills: Must have strong clerical and numerical abilities, including a good understanding of the metric system of weights and measurements. Goodcommunication and organizational skills and attention to detail are also required.
