Provides direct care and oversees follow-up care to individuals
Screenings and referrals as appropriate
Partners with physicians and other health care providers in diagnosis and follow-up
Orders and interprets diagnostic and laboratory tests, develops a differential diagnosis, and develops a treatment plan (including prescribing pharmacologic and non-pharmacologic therapies) for the direct management and treatment
Manages the screening and clinical intervention teams
Consults with managing physician when needed
Collaborates with other health care staff for diagnoses and therapeutic management including: prescribing medications and therapies, patient education, and appropriate referrals for cardiovascular disease and health management program
Maintains knowledge of clinical areas of practice


Masters Level Nurse Practitioner required
Nurse Practitioners must be credentialed through the NUMC credentialing process


Prefer experience working as a care provider in primary care setting
Experience building broad-based coalitions and leading initiatives with diverse stakeholders and complex clinical and community initiatives groups required
Experience supervising professional and administrative staff

Demonstrated leadership and management skills and the ability to work effectively and drive consensus amid diverse stakeholders

MN RN License
Advanced Practice Nurse Certification by ANCC
Minnesota Prescriptive authority and DEA licensure
BCLS certification; ACLS suggested

Allina Health does not hire new grads from all academic institutions. Many proprietary on-line schools do not meet Allina Healths standards due to the minimal oversight of the students clinical experience, the high faculty/student ratio, and the lack of focus on national certification standards in the curriculum.

Work Schedule

Mon-Fri: 8am-5pm; daytime shift. Potential for coverage of extended hours clinic, which could include some evenings (until 7pm) and some Saturdays (limited hours TBD; 3-4 hours)
Location: Springfield/Lamberton clinics and may also work at New Ulm Medical Center
