Coordinates assessment, referral, and intake of patients entering the Advanced Heart Failure Program
Coordinates care of advanced heart failure / pulmonary hypertension patients in the clinic setting
Coordinates care during transition from inpatient to outpatient setting, including periodic telephone assessment
Coordinates long-term outpatient care, collaborating with providers to ensure that patient;s needs are addressed
Monitors patient diagnostic data including assessment of data from home monitoring devices
Organizes systems of care for heart failure and pulmonary hypertension populations
Graduate of accredited school of nursing
BSN or higher degree preferred
3-5 years experience with heart failure and/or pulmonary hypertension patients. Telemetry or ICU experience, and/or prior clinic and outpatient experience preferred
Demonstrates knowledge and skills in the care of patients with heart failure, with demonstrated clinical expertise and increasing levels of responsibility and independence
Skills in the areas of interpersonal communication and conflict management, problem solving, decision making and critical thinking are highly desirable
Experience with acutely ill patients, clinic and outpatient management, and clinical research trials highly desirable
Experience with computer applications desirable
Current Minnesota RN license
Clinical Transplant Coordinator (CCTC) and/or advanced nursing practice certification preferred
Achieves CCTC within 2 years of hire (preferred)
Monday to Friday business hours, with flexibility to accommodate occasional early meetings and/or late days if necessary.
