The Nurse Practitioner works to establish collaborative processes that promote quality and cost effective care that optimizes/enhances the physical and psychosocial health of patients across the health care continuum. The Nurse Practitioner is responsible for making decisions that impact health care outcomes, resource allocation and customer satisfaction. Provides care management support to panel of patients who require specialty services integrated and in support of their overall plan of care. Interacts with and supports the specialty care provider and interdisciplinary care team.

Practice Responsibilities

The Nurse Practitioner works as a part of a physician/nurse practitioner team, providing and coordinating care for the patient across the continuum (outreach sites - clinic - hospital).
The Nurse Practitioner is responsible for ensuring continuity of patient care in all settings.
The Nurse Practitioner works in collaboration with other Oncology team members.

Clinical Practice Responsibilities
Performs and documents history and physical examination based on age and history on complex and chronically-ill patients.
Performs independent follow-up of patients in the clinic setting.
Prepares orders as appropriate on for cancer surveillance monitoring.
Formulates the appropriate differential diagnoses based on the history, physical exam, and clinical findings.
Formulates a plan of care to address complex, acute and chronic health care needs.Integrates knowledge of pathophysiology that pertains to acute and chronically ill patients to assist in planning and implementation of treatment.
Synthesizes data from a variety of sources to make clinical judgments and decisions about appropriate recommendations and treatments.
Assesses the impact of an acute, critical and/or chronic illness or injury on the individuals: health status (physical and mental), functional status, including activity and mobility, nutritional status, sleep and rest patterns, quality of life, family, social, and educational relationships.
Assesses patient/family bio/psycho/social needs considering physical and age-specific needs, recognizing the dynamic nature of the patients condition, and provides psychosocial support to patient and family members.
Educates patients/families about self-management of acute/chronic illness with sensitivity to the patients learning ability and cultural/ethnic background. Collaborates with health care team in ensuring that patient/family needs are met.
Determines the need for laboratory and diagnostic studies, initiates orders, and interprets results.Initiates appropriate and timely consultation and/or referral when problem exceeds the nurse practitioners scope of practice and/or expertise.
Employs appropriate diagnostic and therapeutic interventions and regimens with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy.
Promotes collaboration among members of the multidisciplinary health care team to facilitate optimal care for complex acute and chronic patients.
Incorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute and chronic illness.
Prescribes medication in accordance with practice agreement with the collaborating physician.Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
Assesses for multiple interactive and synergistic effects of pharmacological agents, including over-the-counter (OTC) preparations and alternative and complementary therapies, in patients with complex acute and chronic illness.
Facilitates the patient's transition between and within health care settings. Coordinates follow up of patients in collaboration with the physician.
Submits billing levels at time of visit or procedure.Completes consultation, transfer, and survivorship summaries as needed.
Acts as consultant for triage nurses.Performs phone consultation with outpatients.
Acts as a liaison between the oncology providers, providing staff consultation and education as needed.

Serves as role model in promoting the interdisciplinary team approach
Keeps the program team informed of progress on implementing program components.
Serves as liaison between physicians and departments to ensure that timely and high quality care is provided.
Establishes positive relationships with physicians and their office staff as well as hospital staff.
Integrates patient education and resources with other departments in the hospital to ensure consistent information is provided.
Assumes a leadership role in establishing and monitoring standards of advanced practice to improve patient care in collaboration with other experts as appropriate.
Collaborates with physicians and other health care professionals as a participant in multidisciplinary quality improvement teams as assigned.
Accepts responsibility, authority, and accountability as a professional nurse.
Meets expectations for clinical and professional development.
Models practice expectations and shares knowledge with colleagues.
Maintains current knowledge regarding state and federal regulations impacting NP practice.
Utilizes internal resources (e.g. ethics committee, risk management, legal department) and external resources (e.g. professional organizations, government officials, community agencies) to facilitate the resolution of patient advocacy, moral, and ethical issues.
Integrates the nurse practitioner role into systems, processes, and decision making to function fully within the health care team.

Program Development and Promotion

Assists with cancer awareness/prevention activities.
Identifies opportunities for self and team members to participate in speaking engagements.
Provides education to staff about the care of cancer and disease patient care.
Assists with the development, implementation, and maintenance of policies/procedures/standards of care.
Assists with the development and implementation of quality improvement activities.
Stays informed of current initiatives, expectations, and department communications.

Education

Master's degree in Nursing required

Qualification/Skills

Experience
Prefer at least 3-5 years of surgical and oncology Nurse Practitioner experience. Minimum of 1 year of experience required

Skills
Ability to work as part of a physician/nurse practitioner dyad
Excellent teamworking abilities; professional image via appearance and conduct
Ability to communicate effectively with patients, staff, physicians, and the public
Advanced physical and psychosocial health assessment skills
Sound knowledge of adult cardiovascular health, and related diagnostic and clinical therapies
Ability to teach patients, families, staff, and students; knowledge of research methodologies
Competent to function as preceptor
Able to demonstrate ability to deal with high acuity/complexity and rapidly changing situations

Licensure/Experience
Formal education as a nurse practitioner in an area consistent with the patient population served
Current Registered Nurse License for the State of Minnesota
Certification as a nurse practitioner by appropriate professional organization
Minnesota prescriptive authority and DEA licensure

Allina Health does not hire new grads from all academic institutions. Many proprietary on-line schools do not meet Allina Healths standards due to the minimal oversight of the students clinical experience, the high faculty/student ratio, and the lack of focus on national certification standards in the curriculum.

Rotating weekend call 1:6

Position will work at United Hospital in St. Paul, Regina Hospital in Hastings and River Falls Hospital in River Falls Wisconsin
