Practice as a licensed independent practitioner (LIP) in the care of patients in a clinical setting. Practice includes direct patient care, participation in program development and participating in primary care team functions. The Nurse Practitioner works to establish collaborative processes that promote quality and cost effective care that optimizes/enhances the physical and psychosocial health of patients across the health care continuum. The Nurse Practitioner is responsible for making decisions that impact health care outcomes, resource allocation and customer satisfaction. Provides care management support to panel of patients who require specialty services integrated and in support of their overall plan of care. Interacts with and supports the specialty care provider and interdisciplinary care team.
The Nurse Practitioner works as a part of a physician/nurse practitioner team, providing and coordinating care for the patient across the continuum (outreach sites - clinic - hospital).
The Nurse Practitioner is responsible for ensuring continuity of patient care in all settings.
The Nurse Practitioner works in collaboration with other Oncology team members.
Optimizes the collaboration and coordination of the interdisciplinary team to enhance the environment of patient care.
Collaborates with other disciplines in teaching, consultation, management, and research activities to improve outcomes in nursing practice and to enhance the healthcare environment.
Facilitates peer support by acting as a resource to others, and providing constructive, ongoing feedback.
Identifies learning needs, in collaboration with the leadership team, that are validated and prioritized based on evidence and standard of care changes, competency needs, assessment of quality activities, safety and risk management data, and patient population goals
Develops, coordinates, and/or presents educational programs related to area of expertise to meet learners needs
Integrates current practice, new research findings and technological advances into educational programming.
Mentors staff resulting in employee development along a continuum of novice to expert
Helps to create a learning environment that fosters self-learning, reflective practice, critical thinking, technical and interpersonal skill development

Implements strategies to improve care that are appropriate to the complexity, patient, hospital, and community needs, and resources of the system
Develops and implements, as appropriate, programs within clinical areas in collaboration with other health care professionals and community liaisons
Assists development of clinical indicators to evaluate program effectiveness
Supports and champions program goals related to specialty
Influences resource utilization to promote safety, effectiveness, and fiscal accountability in the planning and delivery of patient care
The ONP uses evidence-based clinical practice guidelines when available to guide screening activities, identify health promotion needs, and provide anticipatory guidance and counseling, addressing environmental, lifestyle, and developmental issues.
Assesses patients on admission to the hospital, and in the Oncology clinic. Performs and documents history and physical examination based on age and history on complex and chronically-ill patients
Rounds on hospitalized patients in various settings including acute care to advance the plan of care, reassesses as patient condition warrants. Updates and records changes in health status
Performs independent follow-up of patients in the clinic setting
Formulates the appropriate differential diagnoses based on the history, physical exam, and clinical findings
Formulates a plan of care to address complex, acute and chronic health care needs. Integrates knowledge of pathophysiology that pertains to acute and chronically ill patients to assist in planning and implementation of treatment
Synthesizes data from a variety of sources to make clinical judgments and decisions about appropriate recommendations and treatments
Assesses the impact of an acute, critical and/or chronic illness or injury on the individuals: health status (physical and mental), functional status, including activity and mobility, nutritional status, sleep and rest patterns, quality of life, family, social, and educational relationships
Assesses patient/family bio/psycho/social needs considering physical and age-specific needs, recognizing the dynamic nature of the patients condition, and provides psychosocial support to patient and family members
Educates patients/families about self-management of acute and chronic illness with sensitivity to the patient's learning ability and cultural and ethnic background. Collaborates with health care team in ensuring that patient/family needs are met
Determines the need for laboratory and diagnostic studies, initiates orders, and interprets results. Initiates appropriate and timely consultation and/or referral when problem exceeds the nurse practitioners scope of practice and/or expertise
Employs appropriate diagnostic and therapeutic interventions and regimens with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy
Promotes collaboration among members of the multidisciplinary health care team to facilitate optimal care for complex acute and chronic patients
Incorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute and chronic illness
Prescribes medication in accordance with practice agreement with the collaborating physician; prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
Assesses for multiple interactive and synergistic effects of pharmacological agents, including over-the-counter (OTC) preparations and alternative and complementary therapies, in patients with complex acute and chronic illness
Facilitates the patient's transition between and within health care settings, such as admitting, transferring, and discharging patients; coordinates discharge plan and follow-up of patients in collaboration with the physician
Submits billing levels at time of visit or procedure. Completes consultation, transfer, and discharge summaries as needed
Acts as consultant for triage nurses; performs phone consultation with outpatients
Acts as a liaison between the medical oncologists and outreach sites, providing staff consultation and education as needed
Collaborates with physicians and other health care professionals as a participant in multidisciplinary quality improvement teams as assigned
Maintains current knowledge regarding state and federal regulations impacting NP practice
Utilizes internal resources (e.g. ethics committee, risk management, legal department) and external resources (e.g. professional organizations, government officials, community agencies) to facilitate the resolution of patient advocacy, moral, and ethical issues
Integrates the nurse practitioner role into systems, processes, and decision making to function fully within the health care team
Participates in medical and advanced practice research projects
Integrates research to promote evidence-based practice for patients with complex acute, critical, and chronic illness

Education

Master's degree in Nursing

Qualification/Skills

Experience
Highly prefer at least 3-5 years of oncology/infusion nursing experience

Allina Health does not hire new grads from all academic institutions. Many proprietary on-line schools do not meet Allina Healths standards due to the minimal oversight of the students clinical experience, the high faculty/student ratio, and the lack of focus on national certification standards in the curriculum.
