Job Description

The primary function of this job is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of this facility; to promote quality and cost effective outcomes on the sub acute unit; by serving as a liaison between the caregiver and the patient, the payer and the External Case Manager.


Conducts and coordinates the interviewing of each resident for the MDS (Minimum Data Set).
Notify Unit Coordinators of need for special assessment activities or need to amend the admission process that is identified during the MDS process.
Schedule and track MDS schedule according to the RAI (Resident Assessment Instrument) manual.
Travel between facilities as needed.
As part of the UR (Utilization Review) and Sub-Acute Team, offer input to the admission and discharge process.
Serve as resident and family advocate.
Maintain compliance with DPH, JCAHO and OBRA regulations.
Acts as facilitator of the Interdisciplinary Care Plan Team.
Develops PPS and OBRA assessments of the nursing needs of each resident, utilizing the MDS process as required by current RAI manual, rules, regulations and facility policy.
Ensure that care plans and assessment duties are carried out according to the RAI manual and facility policy.
Serves on and participates in, and attends various committees of the facility (eg. Department Head, Strategic Planning and Marketing, PI, Orientation ) as required, and as appointed by the Administrator.
Recruits, hires and train, schedules department hours, monitors absenteeism, counsels/disciplines, evaluate, check competency and terminates MDS/MMQ staff as requested or necessary: complying with all state and federal regulations.
Assists the In-service Director/Educator in developing any training activities needed concerning MDS assessment/care plans functions (including but not limited to initial or refresher courses relative to techniques for interviewing residents, documentation to support care provided, ADLs ((Activities of Daily Living))psychotropic drug use).
Ensure department is within budget guidelines and the Director of Nursing or Administrator is informed of adjustments needed to ensure accuracy of reimbursement.
Must maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information: and adhere to Residents rights.
Perform other work related/personal care duties as requested.


Must be a graduate of an accredited School of Nursing, A Baccalaureate Degree in Nursing is preferred.
Must be Certified Resident Assessment Coordinator (RAC-CT) by the American Association of Nurse Assessment Coordination for Long Term Care.
Five years of medical-surgical and/or gerontology experience is preferred. At least two years of previous supervisory and/or head nurse experience
Must be currently licensed to practice as a Licensed Nurse in the Commonwealth of MA
Ability to make independent decisions
Leadership and Supervisory Ability
Must have patience, tact, and a cheerful disposition and the ability and willingness to handle difficult patients, families, and situations.
Must be able to lift a minimum of 50 pounds and meet all other Physical and Sensory requirements of the job.
