CASE MANAGEMENT SPECIALIST - WEEKEND OPTION - DISCHARGE PLANNING

Facilitates cost-effective, quality patient outcomes by collaborating with staff and providing guidance, education and support to assigned patients.
Responsible for the coordination and facilitation of the plan of care.
Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needs and support systems.
Collaborates with the Medical Director, Director, Manager, and Supervisor of Case Management on patients who are not meeting the regulatory requirements for hospitalization.
Provides initial and concurrent reviews for approval of hospital stay for all patients. Notifies patients when stay has been denied.
Collaborates with the interdisciplanary team to ensure patient quality of care and safety of discharge.
Provides education to physicians, staff and patients/families.
Adheres to all organizational policies and procedures. Associate's degree or higher in Nursing or Diploma in Nursing required. Bachelor's degree or higher in Nursing required within 5 years' of employment in this position.
Licensed as a Registered Nurse (RN) required. Eligible to practice Nursing in the State of Kentucky required. Certified as a Basic Life Support (BLS) Healthcare Provider preferred. Certified as an Advanced Cardiovascular Life Support (ACLS) Healthcare Provider preferred.
A minimum of 2 years' progressive experience in a related field required. Experience in Case Management preferred.

Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
