At the Visiting Nurse Association (VNA) of Cape Cod, our goal is to provide comprehensive home health and hospice services that enable individuals to receive care in the privacy and comfort of their own homes. The VNA provides an array of health services from skilled nursing and therapies in the home to community health programs for patients from Plymouth to Provincetown and the islands. The Clinical Team Manager has the responsibility for organizing, coordinating, implementing, and evaluating the care management as well as the safety of patient care in designated area(s).
In collaboration with the Human Resources Department reviews application, interview and hire employees.
Assist with the orientation and preceptor program for all new direct reports.
Coach, support, and monitor progress of all new direct reports.
Organizes, directs, coordinates and evaluates the activities related to patient management. Assess this data utilizing tools and reports designed and developed by Business Analyst as well as or clinical software (SHP and Delta).
Educate and evaluate staff in case management responsibilities and in the utilization of evidence based practice through caseload review.
Ensure accurate diagnostic coding, and OASIS data congruent with patient profile and plan of care.
Ensure staff is meeting productivity.
Provide clinical expertise and serve as a resource to staff (including CHHAs) and community.
Conduct team meetings and interdisciplinary case conferences.
Routinely rotates through clinical management on-call responsibilities to ensure proper patient/staff safety.
Ensure weekend/holiday on-call and staff coverage to meet agency need.
Provide direct care as indicated by agency need.
Complete performance evaluations in a timely manner at 4 month, annual or as needed.
Work in collaboration with Human Resources to discipline clinicians through education, corrective actions plans, verbal/written warnings and or discharge as indicated.
Ensure compliance with all governmental and DPH clinical guidelines, conditions of participation, grant requirements and JCAHO accreditation.
All other duties as assigned.
Current license in Massachusetts in field of expertise.
CPR is required.
BS or Masters in field of expertise preferred.
Minimum three (3) years clinical experience including two (2) in homecare/community health required.
Computer skills preferred.
