Job Description

The Elder Service Plan of the Cambridge Health Alliance is a Program of All Inclusive Care for the Elderly (PACE). ESP of CHA serves adults 55 years old and older with chronic medical and mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. ESP of CHA's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.

The Dietitian is a member of the ESP Interdisciplinary Team who provides overall assessment and management of participants nutritional needs including assessments, counseling, planning nutritional meals and collaborating with the IDT in the participant's overall plan of care. The dietician demonstrates expertise in assessment, clinical decision-making and clinical practice. The dietician also works with the participant and IDT regarding chronic disease management, making recommendations and providing interventions that enable frail elders to live safely in the community. Travel to participant's homes required.


M.S., R.D.

Valid MA license

Computer literacy; Epic preferred

1 years experience in geriatrics required

1-2 years in ambulatory setting strongly preferred
