JOB TITLE: Home Health & Hospice Physical Therapist

I. POSITION SUMMARY:

Responsible for evaluation, planning, directing, and administering physical therapy treatment as prescribed by a physician in an effort to restore function and/or prevent disability following injury, disease, physical disability. Also assists clients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

II. QUALIFICATIONS:

Current State Physical Therapy Licensure, graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.

Current BCLS (CPR) certification.

One or more years of previous experience in all aspects of physical therapy.

Valid state drivers license, current automobile insurance and reliable automobile.

Ability to read and communicate effectively in English.

Strong written and verbal skills

Working knowledge of Physical Therapy Standards of Care and Professional Practice

Knowledge of physical medicine, physical therapy modalities, anatomy and physiology.

III. ESSENTIAL FUNCTIONS

Performs an appropriate evaluation on assigned clients as related to their condition and establishes a plan of care. Re-evaluates and adjusts treatment plan as indicated.

Observes, records, and reports to physician clients reaction to treatment and any changes in clients condition. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness, including family as appropriate.

Assess the need for equipment and refers to resources, demonstrates the use of equipment to responsible persons.

Maintains appropriate documentation as related to care, including visit notes, orders, summaries, coordination, etc.

Coordinates and communicates effectively with other team members.

Demonstrates adherence to the laws and regulations passed by official regulating bodies, general principles of ethical conduct and internal policies and procedures.

IV. NONESSENTIAL FUNCTIONS:
