Outpatient Staff Pharmacist

Outpatient Staff Pharmacists provide clinical services by insuring appropriate drug therapy with patient consultation and education. They serve as a resource for drug information in a variety of settings including the retail pharmacy, clinic medication therapy management (MTM) services, and the anticoagulation clinic. Outpatient Pharmacists provide direction to supportive personnel (technicians, interns, nurses, and clerks). They must work cooperatively with patients, physicians, nursing staff, hospital pharmacy services, pharmacists, managed care (third party) coordinators and support personnel.
Education and Experience:
Doctorate of Pharmacy degree or Bachelors of Science Degree or equivalent required. Previous outpatient or community pharmacy experience preferred.
Must be a graduate from an accredited college of pharmacy on or after May 1996 or successful completion of an MTM education program (approved by the Board of Pharmacy and the American Council of Pharmaceutical Education). (Not applicable for casual pharmacists)
Current Minnesota pharmacist license. Must complete annual education requirements.
Special Skills and Aptitudes:
Working knowledge of all state and federal regulations governing the practice of pharmacy
Excellent customer service and communication skills required
Good attention to detail and the ability to implement policies and procedures
Must possess computer data entry skills and be able to perform mathematical calculations with an exacting degree of accuracy
Proficient in the English Language
Ability to handle confidential information discreetly and appropriately
Verifies prescriptions for accuracy, appropriate dose/use, and interactions.
Compounds medications using standard formulas and processes and calculates formulas as needed.
Monitors patient profiles to ensure that accuracy, allergies, interactions, duplicate therapies, rational drug therapy, cost efficacy, and incompatibilities are realized so that better patient care is received.
Ensures compliance with facility policies and procedures as well as state and federal regulations governing the practice of pharmacy.
Involvement in maintaining site inventory. (Not applicable for casual pharmacists)
Provide appropriate pharmaceutical care.
Provide direct supervision and training for site staff & interns. (Training not applicable for casual pharmacists)
Assists in teaching programs (departmental, medical, nursing, community, etc) as needed.
Use time effectively and demonstrate ability to prioritize workload.
Demonstrate ability to respond to patient/staff questions or concerns in an appropriate manner.
Attend and participate in staff meetings and in-services. (Not applicable for casual pharmacists)
Understands and follows safety and reporting procedures specifically related to the job duties.
Demonstrates interest in and commitment to assuring patient satisfaction with services provided by the medical campus. (Not applicable for casual pharmacists)
Maintains knowledge of insurance plans and parameters.
Reviews, monitors, and orders drug therapy including dosing adjustments as authorized by collaborative practice agreement(s).
Demonstrates knowledge of pharmacy computer system.
Performing a Can-Do Spirit while other duties may be assigned.
