Provide nutritional screening, assessment, monitoring, follow-up and documentation on all home health patients.
Provide nutrition education including diet education and food drug interactions to home health patients and families in accordance with hospital protocols and policies.
Provide nutritional interventions and recommendations and accurately document in EMR.
Provides in-home assessments and education based on department policies and procedures.
Actively participates in department meetings and committees.
Complete special projects including active participation National Nutrition Month and department in-services and participates in department PI projects.
Bachelor's Degree in Food, Nutrition or related field
Current Registered Dietitian license
Master's Degree in Nutrition or in Public Health Nutrition is preferred.
One year recent experience as a Registered Dietitian in a clinical or public health area.
Registration by the Academy of Nutrition and Dietetics, CDR or Registration eligible
Member of the Academy of Nutrition and Dietetics is preferred.
Familiarity with computers, computerized charting, kitchen function and equipment, excellent communication and critical thinking skills.
Valid Drivers License required.
Schedule: 3 days week, 8a-4:30p, Day shift
