Assess for symptoms and underlying family dynamics to help determine treatment plan
Working with patient, family and treatment team establish plan and goals
Establish therapeutic alliance with the patient and family
Adapt interventions to meet individual or family needs
Demonstrate varied repertoire of therapeutic skills for varied patient and family population
Creates safe therapeutic milieu
Working with treatment team, develop an on-going treatment plan for patient and family
Demonstrate initiatives and processes that support continuous quality improvement in patient care and education
Meet documentation requirements
Maintain communication with patients, family and other treatment team members to optimize success for patient and family
On going coordination of care with community care providers
Consistently monitors the treatment plan, goals and discharge plan to ensure appropriate utilization of resources to meet patient and family needs
Masters degree in clinical social work, family therapy, or counseling related field
Must have demonstrated clinical experience in providing family assessment, group psychotherapy and education to mental health patients in acute crisis
Provide coverage primarily at Mercy Campuses, serving adults in the day treatment and partial hospitalization programs. Occasional cross-system coverage serving adolescents and adults.
