A TriHealth Clinical Pharmacist is responsible for providing optimal pharmaceutical care to all patients we serve through distributive and clinical functions. The Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department?s clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Additional responsibilities include, but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and/or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership
In this role the Ambulatory Pharmacist in Population Health will be to function as part of a Primary Care team that is committed to achieving value-based quality and utilization measures by providing comprehensive medication management services, with a focus on medication and formulary adherence, safe and appropriate drug therapy in chronic conditions and transitions in care, polypharmacy management, and identifying/resolving cost of care opportunities. To be successful in this role, an individual will need to have excellent interpersonal and communication skills and problem solving skills, with a demonstrated ability to interact with a diverse population. Pharmacists in this role must be able to work effectively with a multidisciplinary team and exhibit a professional and positive image when interacting with patients and other providers.
Education: Bachelor's Degree in Pharmacy Pharmacist or Eligible for licensure
Experience: 1-2 years in a related field
Specialized Knowledge: Prescription formula and mathematics for calculating dosage and quantity. Working knowledge of all Ohio Board of Pharmacy regulations.
Licensure / Certification: Current valid State of Ohio Pharmacist license.
