Qualifications


Current licensure as a registered nurse in appropriate state (if NP).
Current licensure as a Nurse Practitioner or Physician Assistant in appropriate state.
Current certification in CPR
National certification, in applicable area.
Successful completion of Essentia Health credentialing process prior to practice.

Our Primary Care team is seeking an experienced practitioner to join our team-based model of care. This position will be based in Family Medicine (with coverage in the Walk-In Care setting as needed) to allow our patient and family-centered care group to further commit to a positive experience with exceptional customer service. With the convenience of the PR clinic being adjacent to CHI St. Joseph's Hospital, we offer an ideal environment to work collaboratively with a team that offers both primary care and many specialty care services right on-site.

Experienced Primary Care practitioner required!(2+ years of advanced practice experience)
Full time, 0.8-1.0 FTE based in Family Medicine with some rotating shifts within Walk-In Care.
Pediatrics is a large portion of patient base, but must be able to care for full scope.
Family Medicine trained to provide healthcare for all ages; will collaborate with other primary and specialty care providers to form team based model.
Minimal oversight; ability to step into a well-established practice setting.
14-20 patients per day
Will provide outreach to Walker and Menahga Clinics to assist with coverage needs on occasion.

The Nurse Practitioner or Physician Assistant utilizes advanced health assessment, knowledge, and decision-making skills, while working both independently and in collaboration with physicians and other health care professionals. The NP/PA will provide health care to individuals and families, emphasizing health promotion and disease prevention, caring for patients ranging in age from newborn to elderly. As a member of the Care Team, utilizing Essentia Care Processes, the NP/PA works collaboratively with other care team members to achieve Care Team objectives, and to provide continuity of patient and family care. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.

EDUCATION/TRAINING:

Masters or Doctorate degree in Nursing or Bachelors or Masters degree in Physician Assistant Studies from an accredited program.
