Serve patients as a full member of a patient-centered mental health center dedicated to helping patients get well and stay well. Provide diagnostic assessment, onsite crisis assessment and crisis intervention, determining ongoing care according to level of clinical ability, referrals, and education services across the continuum of care. Deliver comprehensive team-based health care with improved identification of needs, access to the right care at the right time in the right location, improving outcomes of care, and working to achieve the highest level of recovery and optimal wellness for our patients. Serves as an integral core team member within the inter-professional patient centered team. Important resource for patients and team members for issues related to mental health, chemical health, and lifestyle management to support optimal patient functioning.
Must possess skills that promote excellence in communication, flexibility, team-based care and decision making. Considerable commitment to an inter-professional environment. Follow-up to ensure continuity of care and those patients are receiving appropriate behavioral health management. Has a good understanding of human behavior; be familiar with legal issues, ethical issues, insurance regulations and protocols for proper referral or application for various programs. Must be comfortable providing coverage across all potential age populations.
Must have current licensure and/or certification to provide counseling/therapy services in the state in which the care is provided. Master's Degree in a behavioral science is required to become independently licensed as a Mental Health Professional.
