Under general supervision, implements physician orders and provides patient care to people of all ages by assessing, planning, implementing and evaluating care and plan of care. Accepts accountability for the nursing care of assigned patients based on competency, training, experience and patient need. Supports the mission of Tri-County Health Care and maintains patient confidentiality and privacy compliance. Accepts patient safety as a key area of focus in job performance.
Uses and documents the nursing process to provide individualized patient-focused care from admission to discharge. Serves as a resource for nursing care.
Delegates appropriate responsibilities to other nursing personnel based on their skill, experience and competency level consistent with patient care needs and directs the patient care provided.
Coordinates the nursing plan of care with the medical plan of care in the development and implementation of the Patient Care Plan. On admission establishes and updates Plan of Care daily, uses age appropriate data to develop care plans.
Assists the physician with rounds, physical examinations and procedures. Initiates and implements physician orders.
Maintains updated age-specific competencies for infant, child, adolescent, adult and geriatric patients.
Trains and tests out on competencies annually.
Performs patient care activities to provide for the physical, social, psychological and spiritual needs of the patient/family.
Safely administers treatments, conducts nursing procedures, maintains and safely uses therapeutic equipment to carry out physician orders which relate to the practice of nursing.
Observes, assesses, reports and documents subjective/objective data to maintain an accurate and timely medical record, i.e., patient's condition, reaction to drugs, treatments, significant incidents, medications administered, response to pain management, intake and output, vital signs.
Safely administers medications including: orals, injectables, intravenous solutions and blood components. Follows proper procedure to ensure safe medication administration and considers age and size and individual needs of patients, including pain management. IV administration includes:
Venipuncture and initiation of IV therapy.
Initiating and monitoring IV rates and IV infusion pumps.
Daily maintenance care of IV site, including tubing changes.
Verifying, initiating, monitoring and discontinuing IV's including blood and/or blood components.
Reports all unusual conditions or reactions to RN Supervisor, or to physician as appropriate to assist with safe provision of care.
Establishes and performs ongoing culturally sensitive patient/family teaching and discharge planning to meet the identified goals of the patient care plan. Encourages patient/family participation in assuring a safe hospital experience.
Initiates and/or assists with Basic Cardiac Life Support in cardiopulmonary arrest (when indicated) and maintains CPR and ACLS performance. Uses appropriate age and size related equipment for emergencies.
Maintains knowledge base on available resources and nursing policies and procedures.
Complies and assists others to comply with established policies and procedures for safety and infection control when observing and/or giving direct patient care to ensure the well-being of the patient, family, visitors and staff, i.e., restraints, fire, universal precautions/body substance isolation and transmission based precautions.
Relays and interprets patient data and information to ancillary departments and health agencies, i.e., test requests, transfer sheets, discharge forms, diagnostic study results, to accommodate continuity of care.
Participates in performance improvement initiatives identifies problems and takes action related to opportunities for improvement for informal as well as formal patient needs and processes. Is especially concerned with patient safety and reduction in errors.
Coordinates the nursing plan of care with the medical orders to provide safe ED services. Uses and documents the nursing process to provide individualized patient care. Observes, documents and reports to the physician subjective/objective patient data in an accurate and timely manner.
Assists the physician with physical examinations and procedures in the ED. Administers medications and treatments and carries out the physician orders which relate to nursing and coordinates the provision of other inter-department services to ensure safe quality patient care.
Initiates and/or assists with Basic and Advanced Cardiac Life Support (ACLS) in cardiopulmonary arrests and maintains CPR training, ACLS, TNCC, ENPC and CALS certification.
Conducts and documents patient teaching and discharge instructions with patient, family, significant others and appropriate health care providers to ensure safe continuity of care.
Stocks supplies and maintains equipment for safety and accessibility.
Must maintain active RN Minnesota license
Required: Graduation from a Registered Nurse education program (Associate Degree, diploma, or Bachelor's of Science - Nursing).
