The Oncology Nurse Navigator (ONN) is a professional registered nurse with Oncology specified clinical knowledge who offers individualized assistance to patients, families and caregivers to help overcome healthcare system barriers. Using the nursing process, the ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. Responsibilities: ONN services begin with prevention and screening activities and continue through diagnosis, treatment, survivorship, and end -of- life care. Advocate for quality, cost- effective, patient centered outcomes and facilitate communication between the patient/survivor, caregiver, and interprofessional cancer care team. Educate patients on their diagnosis, treatment options, and the resources available. Consult and collaborate with the healthcare team in the assessment and management of the patients journey through diagnosis, treatment and recovery. Facilitate appointments for consults and support services within the established service standards, including social workers, dieticians, physicians, rehabilitation, etc. Follow patients through the care continuum, including inpatient admissions, and collaborate with inpatient care management resources. :  Under the direction of the Director of Quality Management, oversees various quality-related initiatives within the organization, which may include but is not limited to activities related to continuous quality improvement within the health system.  May also provide support to other members of the Process Improvement Department team (Quality Managers) as needed. Responsibilities: Managing the maintenance, distribution, and analysis of all quality indicator dashboards organization-wide Serving as the organization's ISO Quality Management system backup management representative, to include the performance of internal audits and all related recordkeeping Serve as an ISO internal auditor for non-clinical processes (training will be provided) Participation as an improvement advisor on internal quality improvement committees, to include a focus on data collection, aggregation, display, and interpretation Performing medical record audits for quality-related or risk management-related issues, to include datamining, aggregation of results, determination of optimal methods of displaying quality data, and communication of results on a regular basis to stakeholders. Processing of all organizational administrative policies, to include working closely with members of the organization's management team (including senior leadership) to ensure timely review and approval of policies, any ongoing and necessary revisions, creation of new policies, and posting finalized policies to the hospital policy Portal. Processing and distributing various internal quality reports using the MIDAS+ system. This includes some data entry requirements. Performs ongoing statistical analysis of quality data to identify patterns and trends that may be targeted for improvement. Work closely with members of the hospital's administrative team on analyzing and understanding this data and serve as an improvement advisor as necessary. Maintain physician quality indicator profiles and distribute results on a quarterly basis (Ongoing Professional Practice Evaluation) Provide support to department directors with external database reporting of hospital quality measures Serve as backup liaison to the Director of Quality Management for regulatory survey activities (CMS/Joint Commission/DNV/ODH), to include direct participation in regulatory/accreditation surveys by serving as an escort/guide for surveyors, internal tracer audits, and responding to internal questions/queries on standards interpretation.  Experience with Joint Commission, DNV, CMS, and ODH survey activities is desired. POINT OF CARE TECHNICIAN LABORATORY Position Responsibilities: Draws patient blood samples from neonatal, pediatric, adolescent, adult, and geriatric populations following established aseptic techniques, either within the department or in other patient care areas as necessary including home draws and nursing home patients.  Collects throat and nasal cultures Assists in answering the telephone inquiries Participates in capillary blood collection, billing/registration of non-patient specimens as well as patients coming to out patient to be drawn and/or dropping off specimens, specimen processing and other appropriate assignments.  Collects chain of custody "post accident" urines for Company Care clients and handles random employee urine drug screens for LMH.  Maintains supplies in outpatient and blood draw baskets checking expiration date and documenting in log book. Maintains positive patient/specimen identification and patient confidentiality. Documents instructions to patients for collection of specimen in log book. Requirements: Minimum requirement, high school diploma or GED.  Work requires a high school level of knowledge and the ability to understand laboratory tests offered and the types of specimens required for these tests.
