A Registered Nurse that will coach, consult and teach lactation skills. Make clinical assessments and use current lactation theories and research to develop evidence-based feeding plans that address individual feeding issues while respecting the physician's and family's breastfeeding goals. To work collaboratively with physicians, practice managers, and offices; to capture complete and accurate documentation to assure that the appropriate diagnosis and procedure coding has been assigned; to assess current compliance activities, identify areas of risk, and evaluate risk factors in coding and documentation practices; to develop and implement an education plan to communicate the principles and importance of documentation improvement to members of the professional corporation and other members of the healthcare team; and to serves as a resource on coding related issues. Responsibilities Completes internal audit reviews of patient records on a regular schedule to: (a) evaluate documentation to assign the primary diagnosis, pertinent secondary diagnoses, and procedures for accurate and optimal code assignment and (b) initiate a review worksheet. Conduct follow-up reviews of any areas with identified trends with coding staff. Prompt physicians regarding missing, unclear, or conflicting medical record documentation and obtain additional documentation within the medical record when needed. Collaborate with practice managers and/or staff regarding interaction with physicians on documentation and to resolve physician prompts in a timely manner. Ensure conformity to government-required coding policies and guidelines. Confers with physician coding staff to ensure accuracy of shared records. Participates in the analysis and trending of statistical data for specified patient populations to identify opportunities for improvement.  Assists with chart reviews as needed. Educates practice managers and assigned coder(s) regarding specific documentation needs and coding and reimbursement issues identified through daily and retrospective documentation reviews and aggregate data analysis.  Facilitates change processes required to capture needed documentation, such as forms redesign   Requirements Completion of an accredited two-year program in Medical Records Technology at a technical school or college.  RHIT or CCS credentials preferred. Knowledge of software applications related to Coding (encoder/grouper) is necessary. Excellent organizational, management, planning, interpersonal, written and oral communication skills are necessary.  Ability to work both as a team player and independently. Strong analytical skills. Demonstrated aptitude for quantitative analysis and attention to detail. Able to identify problems and find creative, effective solutions. Able to balance multiple priorities. POINT OF CARE TECHNICIAN LABORATORY Position Responsibilities: Draws patient blood samples from neonatal, pediatric, adolescent, adult, and geriatric populations following established aseptic techniques, either within the department or in other patient care areas as necessary including home draws and nursing home patients.  Collects throat and nasal cultures Assists in answering the telephone inquiries Participates in capillary blood collection, billing/registration of non-patient specimens as well as patients coming to out patient to be drawn and/or dropping off specimens, specimen processing and other appropriate assignments.  Collects chain of custody "post accident" urines for Company Care clients and handles random employee urine drug screens for LMH.  Maintains supplies in outpatient and blood draw baskets checking expiration date and documenting in log book. Maintains positive patient/specimen identification and patient confidentiality. Documents instructions to patients for collection of specimen in log book. Requirements: Minimum requirement, high school diploma or GED.  Work requires a high school level of knowledge and the ability to understand laboratory tests offered and the types of specimens required for these tests. Must be able to pass the National Certification for Phlebotomists Test within two years of being employed.  Continued employment is contingent upon this requirement. POINT OF CARE TECHNICIAN LABORATORY Position Responsibilities: Draws patient blood samples from neonatal, pediatric, adolescent, adult, and geriatric populations following established aseptic techniques, either within the department or in other patient care areas as necessary including home draws and nursing home patients.  Collects throat and nasal cultures Assists in answering the telephone inquiries Participates in capillary blood collection, billing/registration of non-patient specimens as well as patients coming to out patient to be drawn and/or dropping off specimens, specimen processing and other appropriate assignments.  Collects chain of custody "post accident" urines for Company Care clients and handles random employee urine drug screens for LMH.  Maintains supplies in outpatient and blood draw baskets checking expiration date and documenting in log book. Maintains positive patient/specimen identification and patient confidentiality. Documents instructions to patients for collection of specimen in log book. Requirements: Minimum requirement, high school diploma or GED.  Work requires a high school level of knowledge and the ability to understand laboratory tests offered and the types of specimens required for these tests. Must be able to pass the National Certification for Phlebotomists Test within two years of being employed.  Continued employment is contingent upon this requirement. Maintains compliance with safety equipment following OSHA regulations. Maintains certification in CPR. Under the supervision of a Registered Nurse or Physician, provides to individuals and groups nursing care requiring the application of basic knowledge of the biological, physical, behavioral, social and nursing sciences.  Responsibilities Assists Physicians, NP's, RN's in assessment of patients and contributes information in planning and evaluating quality patient care. Provides direct technical nursing care to patients. Administers medications ordered by a licensed provider after having completed the 90-hour Pharmacology Course and passing the National Exam. Maintains excellence in clinical competency and nursing skills through continuing education. Informs Physician, NP, or RN of patient's symptoms, reactions, progress and other information pertaining to patient's care. Maintains a clean, safe and orderly patient, visitor, and staff environment. Practices Universal Precautions. Participates in the ongoing quality improvement program and responds to results obtained from data collection. Requirements Graduate of accredited Practical Nursing program maintains current licensure in State of Ohio. Functions within the scope of practice of nursing for a licensed practical nurse as defined by the Nurse Practice Act. Maintains BCLS certification Possess basic analytical ability to perform assigned tasks and gather and interpret data in situations that are not difficult or complex. Demonstrates ability to provide care for pediatric, adolescent, adult and geriatric patients under supervision of a licensed physician, NP and/or RN. Demonstrates knowledge, skill and ability to perform: phlebotomy, EKG, vital signs, medication administration by oral, med nebulizer, IM, SQ, routes for children and adults, ear lavage, monofilament, glucometer, occult blood, visual and auditory acuity, strep screen, urinalysis, urine pregnancy, specimen collection and preparation as applicable to the practice setting. Demonstrates ability to proficiently operate various office machines including office computer systems (IDX, EMR, Meditech), fax, telephone, photocopier.
