Performs the services of copying, chart assembly, analysis, and chart retrieval of patient medical records.   ESSENTIAL FUNCTIONS: Receive and process patient records following discharge. In an expedient, efficient manner, goes to the various units, picks up the prior day’s discharges, and deposits the records in the coding area. Via computer reports, validates each record into the MRD post discharge: locates and secures any missing paperwork. Ensures all prior day discharges have been received and retrieves any missing paperwork.  Accurate assembly/analysis (A&A) of records following discharge. Accurately analyzes the incomplete record to ensure compliance with regulatory standards and Medical Staff Bylaws, Rules & Regulations. Physical therapy services are provided in response to a physician’s referral, in accordance with the laws governing the state of New Mexico, and within the scope of the therapist’s training, knowledge, and experience. This may include, but is not limited to, evaluation and treatment of functional mobility, motor function, muscular strengthening, sensation, wounds, ambulation, balance, social, and environmental needs. Direct patient care is provided to individuals of all ages, as his or her caseload demands. Performs billing, insurance follow-up, and collection responsibilities for Hospital and physician practices with minimal supervision.   ESSENTIAL FUNCTIONS: Answer phone calls Provides outstanding customer service to patients inquiring about their accounts. Confirms patient insurance eligibility and/or benefits. Audits charts for accuracy of billing/payment/income. Correct address information as required for guarantor, patient, insurance companies, employers etc.  Communicate those changes as appropriate. Discuss and educate guarantors and patients on various services offered by AGH and Clinics as well as methods of payment for those services including but not limited to Medicare, Medicaid, Insurance, Financial Assistance and other state and county assistance programs. Will manage the appeals and denials process to ensure claims are properly resubmitted and reprocessed for payment. Will work outliers, catastrophic claims to ensure additional payment is received.
